What is an IUI and how is it done?
An IUI -- intrauterine insemination -- is performed by placing a very thin flexible cannula (tubing) through the cervix (neck of the womb) and injecting washed sperm directly into the uterus (womb). The whole process takes about 5 minutes - it usually only requires the insertion of a speculum and then the cannula. Sometimes when the cervix is hard to reach, an instrument called tenaculum will be used to hold the cervix, which makes the process a bit more uncomfortable. A typical canulla is shown below.
Where is the sperm collected? How long before the IUI?
Usually the sample is collected through ejaculation into a sterile container. Ejaculate is usually collected in the laboratory in a private setting. It is then given to the laboratory technician who will wash the sperms / semen. This process usually takes between 30 minutes to an hour.
When is the best timing for an IUI?
Ideally an IUI is performed about 36 hours after hCG injection ( medication to mature the eggs and thus leads to ovulation).
The egg is only viable for a maximum of 24 hours after it is released.
What is the success rate for IUI?
The chances of success vary between 5 to 25% per cycle. The low statistics are with one follicle but if there are multiple follicles, the success rate will be as high as 25%. Higher sperm counts will also increase the chances of conception. The rate of multiple pregnancies is 15 to 25 %.
What does an IUI feel like?
IUI is a fairly painless procedure- about the same as having a pap smear done. There can be some cramping feeling afterwards, but often what is felt is ovulation-related rather than from the IUI.

Do I have to lay down after an IUI?
You will lay down on the couch for 15-30 minutes after the procedure.
Do I need to take it easy after an IUI?

A count above one million washed appears necessary for success, with a significant reduction in pregnancy rates when the inseminated is count is lower than 5-10 million Higher success rates are with washed counts over 20-30 million, while increasing counts over 50 million did not appear to offer advantage.
How many IUIs should I try before moving on to IVF?
It depends on your situation or your previous history and also your age. The doctor may advise 2 IUIs on Clomid before moving on to injectables, then do another 2 on injectables. If there is no success after 3 to 4 good ovulatory cycles either on Clomid or injectables , then you would be advised to consider IVF.
Is bleeding common after an IUI?
It doesn't usually happen, but it isn't uncommon.
How long after IUI should implantation occur?
Implantation generally takes place 6-12 days after a well-timed IUI.

What kind of monitoring is usually done for an IUI cycle?
You will need to be seen by your doctor at least twice a weeks prior to the IUI in order to check for the follicular growth, its number and the condition of the endometrium ( the lining of the womb).
At what size are follicles considered mature?
The follicles are deemed to be mature when the size is 16mm and above. Most of the time, the doctors will only prescribed hCG when the follicles are between 18-22 mm. The use of hCG induces ovulation in about 95 percent of women, and will get most mature follicles to rupture.
What are the risks involved in IUI?
The main risks are some discomfort such as cramping, minor injury to the cervix that leads to bleeding or spotting, or introduction of infection. There are also risks of hyperstimulation associated with the use of ovulation induction medications such as clomiphene citrate (low risk) and gonadotropin therapy (higher risk).
Can I take pain medications before or after the procedure?
Most women don't need medication for pain associated with IUI. If there is cramping, panadol (paracetamol) is considered safe.
